Community Care Case Manager

Position Overview

We are looking for a dedicated Case Manager to become an integral part of our team, focusing on assisting elderly individuals in the community to maintain their independence and dignity within their own residences.

Your main duty will involve overseeing a caseload of elderly clients who are part of Home Care Packages. This includes evaluating their needs, crafting tailored care plans, and managing budgets in line with program standards to ensure that each client's specific needs are addressed. You will also be responsible for onboarding new clients and proactively engaging with existing ones using a person-centred approach. Effective collaboration with our Clinical, Scheduling, and Service Delivery teams will be essential for providing comprehensive care.

This is a permanent full-time position that offers the flexibility to organize your own weekly schedule while working independently from home and within the community. Regular visits to clients in their homes will be required, so being located in or near the relevant regions is preferred.

Candidate Profile

To excel in this role, you should possess:

  • Experience in community-based case management (preferred)
  • Familiarity with Home Care Packages (preferred)
  • A valid driver's license
  • A dependable vehicle with at least Third Party insurance
  • Strong verbal and written communication skills
  • Demonstrated empathy and compassion
  • Excellent problem-solving and critical thinking abilities
  • Strong organizational and time management skills
  • Cultural awareness and the ability to engage with diverse populations
  • Advocacy skills to ensure the needs of clients are met
  • Teamwork skills to collaborate effectively with interdisciplinary teams
