About the Role

We are seeking a highly skilled and compassionate Therapeutic Case Manager to join our Intensive Therapeutic Care team at Life Without Barriers. As a key member of our team, you will play a vital role in providing quality case management support to children and young people residing in our therapeutic care homes.

Key Responsibilities
  • Develop and Implement Therapeutic Case Plans: Work closely with children, young people, and their support networks to develop and implement individualized therapeutic case plans that meet their unique needs and goals.
  • Build and Maintain Relationships: Foster strong, supportive relationships with children, young people, and their families to ensure positive outcomes and achieve case plan goals.
  • Collaborate with Teams: Attend shifts within intensive therapeutic care homes and work collaboratively with the team to provide high-quality support and care.
  • Coordinate and Network: Coordinate and network with other agencies to effectively achieve case plan goals within tight timeframes.
  • Ensure Compliance: Ensure compliance with all internal policy and procedure, statutory, and contractual requirements in relation to the management of service delivery.
Requirements
  • Relevant Qualifications: Relevant Bachelor's degree or a relevant diploma and be working towards a degree. (A qualification is desirable but not essential for Aboriginal and/or Torres Strait Islander applicants with relevant experience and a willingness to undertake further training)
  • Case Management Experience: Previous case management experience within an Intensive Therapeutic Care or Out Of Home Care (OOHC) setting.
  • Supporting Children and Young People: Experience supporting children and young people with disability, who have experienced backgrounds of trauma, abuse, and/or neglect.
  • Understanding of Trauma: Understanding of trauma and its effects on children and young people.
  • Current Drivers' Licence: Current drivers' licence.
