About the Role

We are seeking a skilled Case Manager to join our Intensive Therapeutic Care team. As a key member of our team, you will provide high-quality case management support to children and young people residing in our therapeutic care homes. Your primary responsibility will be to develop and implement therapeutic case plans that meet the individual needs and goals of our clients.

Key Responsibilities
  • Develop and implement high-quality therapeutic case plans for children and young people in our care.
  • Build and maintain functional and supportive relationships with children, young people, and their families to support positive outcomes.
  • Attend shifts within the intensive therapeutic care homes and work closely with the team to ensure seamless service delivery.
  • Meet with clients and family members to gather input for case plans and ensure their views are represented.
  • Coordinate and network with other agencies to effectively achieve case plan goals within tight time frames.
  • Participate in an on-call roster as required.
  • Ensure compliance with all internal policy and procedure, statutory and contractual requirements in relation to management of service delivery.
Requirements
  • Relevant Bachelor's degree or a relevant diploma and be working towards a degree.
  • Previous case management experience within an Intensive Therapeutic Care or Out Of Home Care (OOHC) setting.
  • Experience supporting children and young people with disability, who have experienced backgrounds of trauma, abuse, and/or neglect.
  • Understanding of current casework practices for children and young people.
  • Understanding of trauma and its effects on children and young people.
  • Current driver's license.
