Job Summary

We are seeking a highly skilled and experienced Senior Case Manager to join our team. The successful candidate will be responsible for managing the day-to-day operations of our franchise, including client assessments, care planning, and service delivery. This is a key leadership role that requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Manage the day-to-day operations of the franchise, including client assessments, care planning, and service delivery
  • Develop and implement individualized care plans that meet the unique needs of each client
  • Collaborate with clients, carers, and other healthcare professionals to ensure seamless care delivery
  • Monitor and evaluate the quality of care provided and make recommendations for improvement
  • Lead and manage a team of care professionals to ensure high-quality care delivery
Requirements
  • Minimum 2 years' experience in a case management or care coordination role
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Strong leadership and management skills
  • Ability to develop and implement individualized care plans
