We are seeking a dedicated Care Advisor to join our team in South West Sydney. As a key member of our team, you will be responsible for ensuring that our clients receive the highest level of care and support.
About the RoleThe Care Advisor will be responsible for providing comprehensive case management services to clients in the South West Sydney region. This will involve assessing client needs, coordinating with internal and external service providers, and ensuring that all case management activities adhere to organisational policies and legislative requirements.
The successful candidate will have a Certificate IV in Aged Care or equivalent, and previous experience in home care or community services. They will also have intermediate computer skills and a current NSW driver's licence.
Key Responsibilities- Client-Centered Care: Provide comprehensive case management services to clients, ensuring that their physical, clinical, emotional, spiritual, and social needs are met.
- Service Optimization: Assess and identify client needs, coordinating with internal and external service providers to deliver appropriate services and products.
- Compliance & Documentation: Ensure all case management activities adhere to organisational policies and legislative requirements, and maintain accurate and timely documentation.
- Relationship Management: Foster and maintain positive relationships with teams, health professionals, clients, and other stakeholders to enhance the profile of Anglicare At Home.
- Financial Management: Assist clients in managing their care budgets, ensuring accurate documentation of services for correct allocation and accounting.
- Certificate IV in Aged Care or equivalent.
- Previous experience in home care or community services.
- Intermediate computer skills, including proficiency in MS Office.
- Current NSW driver's licence.
