About the Role

This position offers an exciting opportunity to join the New York State team as a Community Health Nurse Specialist. The successful candidate will work within a treatment team providing nursing services to individuals diagnosed with developmental disabilities and their caretakers/families.

Key Responsibilities

  • Visit individuals in their homes to identify factors contributing to their disability and provide recommendations for care.
  • Develop alternative treatment plans and teach caretakers/family members methods of dealing with emergencies.
  • Provide advice on available medical, social, and housing facilities in the community.
  • Intervene and administer emergency treatment as needed.
  • Engage in various treatment modalities, including health teaching, individual and group psychotherapy, family therapy, or milieu therapy.
  • Work with community agencies to ensure a coordinated treatment plan.
  • Supervise and counsel paraprofessional and professional personnel in health-related activities.
  • Participate in staff development and teaching programs.

Additional Responsibilities

  • Function as a Registered Nurse 2 and Charge Nurse for Nursing Triage.
  • Act as a triage nurse, providing guidance to direct care staff/LPNs as needed.
  • Perform other duties assigned by the agency.
  • Participate in agency committees.

About the Salary

The estimated salary for this position is $93,000-$105,000 per year, depending on experience and location.
