Job Summary: We are seeking a skilled Registered Nurse 2 to join our team in the community setting, providing expert care to individuals with developmental disabilities.
Key Responsibilities:
• Provide direct nursing services to individuals in the community, including home visits and care planning.
• Collaborate with treatment teams to develop and implement individualized care plans.
• Conduct assessments and provide interventions to support individuals with developmental disabilities and their families.
• Work with community agencies to ensure coordinated care and services.
• Supervise and counsel paraprofessional and professional personnel in health-related activities.
• Participate in staff development and teaching programs to enhance professional growth and development.
• Function in the role of Registered Nurse 2.
• Function as the Charge Nurse for Nursing Triage.
• Act as a triage nurse, accepting calls from the six-county catchment area and providing nursing guidance to direct care staff/LPNs, as needed.
• Perform other duties as assigned, details to be discussed during interview.
• Participate in agency committees.
• Other duties as assigned. Caseload will include healthcare management of Individuals residing in State Operated sites (IRA's and/or family care setting) and/or Day Habilitation nursing oversight.
Requirements:
• Must be licensed and currently a registered professional nurse in New York State;
• Two years of post-licensure clinical nursing experience, with one year of experience providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.
• Bachelor's degree in nursing may substitute for one year of general post-licensure clinical nursing experience;
• Master's degree in nursing may substitute for two years of general post-licensure clinical nursing experience.
Preferred Qualifications:
• Permanent Registered Nurse 2 or eligible for transfer under Section 70.1 of the Civil Service Law;
• Non-Competitive appointment may be considered.
