
Caring Advocate for Vulnerable Adults

Delivering exceptional care to older adults requires a unique blend of skills, experience, and passion. We are seeking a highly motivated and compassionate individual to join our team as an Older Persons Support Coordinator/Social Worker.
This role presents a rare opportunity to make a meaningful difference in the lives of vulnerable individuals and their families. By working collaboratively with healthcare professionals and community services, you will provide high-quality support and advocacy to those at risk of elder abuse.
The successful candidate will possess a strong understanding of community resources and legislation relevant to elder abuse and family violence. They will be able to develop and implement individualized care plans that address the complex needs of older adults, conduct thorough assessments, and provide counseling and support to clients and their families.
In addition to your technical expertise, you will bring a warm and empathetic approach to your work, ensuring that every client feels heard, validated, and empowered to take control of their lives. Your ability to communicate effectively with people from diverse backgrounds and cultures will be essential in building strong relationships and achieving positive outcomes.
To be successful in this role, you will need to demonstrate excellent assessment and intervention skills, as well as a deep understanding of the social determinants of health and their impact on older adults. You will be able to work autonomously, prioritize tasks effectively, and maintain accurate records and reports.
Key Responsibilities:- Provide high-quality care and support to older adults, their families, and carers
- Work collaboratively with healthcare professionals and community services to respond to suspected elder abuse
- Develop and implement individualized care plans that address the complex needs of older adults
- Conduct thorough assessments and provide counseling and support to clients and their families
- Bachelor degree or qualifying masters in social work
- Postgraduate studies are highly regarded and may be eligible for a higher education allowance
- Valid Victorian driver's license
- Strong understanding of community resources and legislation relevant to elder abuse and family violence
