Our organization is seeking a skilled professional to fill the role of Aged Care Assessor. As an Aged Care Assessor, you will play a vital part in ensuring seniors and their families receive the support they need to maintain independence and well-being.
Key Responsibilities:
- Conduct thorough assessments of older individuals to determine their care needs, preferences, and eligibility for aged care services.
- Collaborate with clients, families, and care providers to develop personalized care plans that cater to the individual's physical, emotional, and social needs.
- Ensure all assessments are completed in accordance with current legislation, policies, and quality standards.
- Work closely with other health professionals, government agencies, and service providers to coordinate services and deliver comprehensive care.
- Advocate for the rights of older adults and ensure their voices are heard throughout the assessment and care process.
- Monitor and review care plans to guarantee they remain relevant and effective in meeting the changing needs of clients.
To be successful in this role, you will require:
- Experience or qualifications in Aged Care, Nursing, Social Work, or a related field as per supporting documents.
- Relevant certifications and qualifications.
- A proven track record of clinical experience.
- Exceptional communication, empathy, and assessment skills.
- In-depth knowledge of aged care services, policies, and regulations.
- A commitment to delivering person-centered care and advocating for older adults' rights.
- The ability to work independently and as part of a multidisciplinary team.
