Job Description
  • Conduct thorough assessments and care planning to support customers at home.
  • Maintain ongoing monitoring of health needs, providing guidance and coordinating with healthcare professionals as required.
  • Effectively manage service delivery within individual plans and budgets.
  • Collaborate closely with customers and their support networks to review and adjust plans, ensuring their needs are consistently met.

This is a part-time (0.8FTE) and full-time position available, offering hybrid working conditions. The successful candidate will work from our Ferntree Gully office and travel when necessary.

Required Skills and Qualifications
  • Experienced Case Manager, qualified as a Nurse, with advanced skills in assessment and clinical care planning.
  • Confidence in working across multiple IT systems and effectively engaging individuals and families to build positive relationships.
  • Previous experience providing clinical care in home, community, or aged care settings for older adults and their caregivers.
