Adult Social Work Review Team Member

Our team is seeking a skilled and dedicated Social Worker to join our Adult Review Team. As a key member of this team, you will play a vital role in completing scheduled and targeted Care Act reviews.

Using a strengths-based approach, you will assess ongoing needs and outcomes for adults in need of care and support. You will work collaboratively with other team members to identify and address the needs of service users.

The ideal candidate will have post-qualified experience as a Social Worker and a degree in Social Work or equivalent. They must also be registered with Social Work England and eligible to work in the UK.

Key Responsibilities:

  • Conduct scheduled and targeted Care Act reviews using a strengths-based approach.
  • Assess ongoing needs and outcomes for adults in need of care and support.
  • Collaborate with other team members to complete reviews and identify service user needs.
  • Carry out Mental Capacity Assessments and Best Interests decisions.
  • Participate in duty rota responsibilities and contribute to team efforts.
