Supporting Young Women at Risk of Homelessness

We are seeking a dedicated and compassionate Case Manager to join our team in supporting young women aged 16-25 who are at risk of or experiencing homelessness in the Southeast of Melbourne.

Your role will be to provide high-quality case management services, including flexible support services and coordination with clients, using a strength-based and trauma-informed approach. You will work closely with clients to understand their needs and develop collaborative case plans that maximize independence and minimize service dependence.

This position requires a strong understanding of youth housing and homelessness policy, context, and systems, as well as experience in supporting people with complex challenges and support needs. You must also have excellent interpersonal skills, verbal and written communication skills, and the ability to maintain confidentiality.

About the Role
  • Provide comprehensive assessments of clients' needs to develop collaborative case plans.
  • Approach and build relationships with external service providers across metro and surrounding suburbs of Melbourne to receive and complete direct referrals.
  • Deliver a comprehensive case management service to clients, including assistance with accommodation, advocacy, assessment, referral, and development of community supports.
  • Assist clients to maximize their options and potential by facilitating involvement in community and social activities, recreational and educational opportunities.
  • Advocate on behalf of clients where agreed, liaising with mental health, drug and alcohol services, legal institutes, government, and non-government service providers.
What We're Looking For
  • Experience in client-facing human services, social work, or other relevant disciplines, with a preference for an understanding of youth housing and homelessness policy, context, and systems.
  • Tertiary qualifications in Social Work, Psychology, or a related discipline highly desirable.
  • Strong initiative and analytical skills, including well-developed organizational and time-management skills.
  • Demonstrated capability in verbal and written communication, and excellent interpersonal skills.
  • A valid Victorian driver's licence is mandatory.
  • Understanding of Child Safe Standards and commitment to upholding them in the workplace.
  • Ability to work collaboratively with other organizations to achieve client outcomes.
