Job Title:
Homeless Services Coordinator

About the Role

This role involves working as part of a multidisciplinary team to provide intensive support services to young people experiencing homelessness or at risk of becoming homeless. The role will cover the North West region and be based across Coburg North, North Fitzroy and Collingwood sites.

The successful candidate will provide flexible and assertive case management responses to clients with multiple and complex needs, conduct comprehensive assessments of a client's needs and circumstances, and work collaboratively within an integrated model to provide case management support.

Key Responsibilities:
  • Provide a flexible and assertive case management response to clients with multiple and complex needs
  • Conduct comprehensive assessments of a client's needs and circumstances both at the point of intake and ongoing through the support period
  • Work collaboratively within Youth Services Integrated Model, having access to all programs and providing case management support
  • Provide intensive case management support to a caseload of young people who are experiencing high and complex needs
  • Work alongside the youth services team at the Sunshine youth hub to provide an assertive response to young people presenting in crisis
  • Provide secondary consult to other professionals and professional care teams
Required Skills and Qualifications:

The successful candidate will have a degree qualification in social work or a related field and relevant experience in a social service environment. They will also have knowledge of youth homelessness, crisis intervention, and the ability to collaborate with other services within the North West region.
