About the Opportunity

We provide high-quality, evidence-based services to prevent or reduce the impacts of homelessness.

Case managers work with young people aged 16-24 who are experiencing homelessness, providing best-practice support and guidance to achieve housing stability and broader goals.

Your Role and Responsibilities
  • Support a caseload of young people through a framework that prioritizes healing and well-being
  • Develop advanced case planning and interventions to address complex needs
  • Assist young people to access ongoing accommodation options and support services
  • Maintain up-to-date knowledge of programs and service providers
Requirements
  • Qualification in social work, youth work, community development or a related field
  • Demonstrated experience working within a case management framework
  • Understanding of mobile and assertive outreach interventions
  • Experience working with young people at risk or experiencing homelessness
  • Strong passion for working with young people and ability to engage actively
  • Excellent communication and problem-solving skills
