Job Description

The Youth Homelessness Services division delivers a range of services to support young people aged 16-24 who are experiencing homelessness. The goal is to provide holistic guidance and best-practice support to achieve housing stability and broader goals.

Youth Intensive Case Managers play a crucial role in this process, providing high-quality intensive case management services to young people presenting to our organization. This involves working with young people to develop individualized case plans that address their specific needs and circumstances.

  • Develop and maintain an up-to-date knowledge of our programs and relevant external service providers to ensure an integrated service response to young people.
  • Build strong relationships with young people, actively engaging them in the case management process and ensuring their needs are met.
  • Work collaboratively with internal and external stakeholders to access specialist support services, including health, mental health, AOD, and therapeutic interventions.

Key Responsibilities:

  • Maintain accurate and detailed records of case work, including progress notes, risk assessments, and safety plans.
  • Participate in regular team meetings and contribute to the development of program initiatives and improvements.
  • Engage in ongoing professional development to enhance skills and knowledge in areas such as case management, trauma-informed practice, and youth work.
Required Skills and Qualifications

To be successful in this role, you will require:

  • A qualification in social work, youth work, community development, or a related field, or extensive experience (4 years) working in the community services sector.
  • Demonstrated experience working within a case management framework, with a focus on supporting young people experiencing homelessness.
  • Excellent communication and problem-solving skills, encompassing interpersonal, verbal, and written negotiation skills.
