As a care professional, you will provide personalized support to clients in their homes.

About the Role

Our team is seeking an individual who can deliver holistic, client-focused care according to each client's tailored care plan.

  • Assist with daily activities and personal care as outlined in the Client Care Plan.
  • Work closely with Registered Nurses to deliver high-quality, client-specific care, including personal hygiene, nutrition, mobility, and more.
  • Identify client care needs and collaborate with the client's care team to ensure exceptional care.
Key Responsibilities
  • Communicate effectively with clients, family members, and colleagues.
  • Support clients with daily living tasks, including laundry, cleaning, and meal preparation, to promote independence.
  • Maintain accurate records of client interactions and report any concerns promptly to the RN/Care Manager.
  • Support clients with mobility aids and range-of-motion exercises under the supervision of an RN.
