Seeking an experienced Bereavement Service Coordinator to join our team at Northern Sydney Local Health District.
Job Purpose:
We are seeking a skilled and compassionate professional to drive the operationalisation of a public health model of care for bereavement services.
Key Responsibilities:
- Coordinate a comprehensive bereavement service for all family members of patients who access our service.
- Provide direct support to family members and carers of patients who have accessed our service.
- Engage with community stakeholders in establishing a bereavement community of practice.
- Implement initiatives focusing on service development, referral pathways, capacity building, and coordination of services.
Requirements:
- Social Worker eligible for membership of the Australian Association of Social Workers with demonstrated experience as level 2 or equivalent.
- Experience as a counselling clinician in bereavement, palliative care, or end of life contexts and specialist knowledge of bereavement, loss, and grief processes.
- Proven ability to work independently with minimal supervision and as a collaborative member of a multi-disciplinary team.
