Mental Health Grief Counsellor

Palliative care bereavement counselling involves providing emotional support to individuals and families coping with the loss of a loved one.
This role requires a high level of empathy, understanding, and effective communication skills to help patients navigate their grief and come to terms with their new reality.
The palliative care bereavement counsellor will work closely with healthcare professionals to deliver comprehensive care that addresses the physical, emotional, and spiritual needs of patients.
- Assess patient's mental health needs and develop individualised care plans to promote optimal wellbeing.
- Provide emotional support and guidance to patients and their families through the grieving process.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and delivery.
- Conduct regular assessments and reviews to monitor patient progress and adjust care plans as needed.
- Deliver evidence-based palliative care services to patients with complex mental health needs.
- Work in partnership with patients, families, and carers to identify and address their unique requirements.
- Develop and maintain strong relationships with healthcare professionals to ensure comprehensive care is delivered.
- Participate in quality improvement initiatives to enhance patient outcomes and service delivery.
- Current registration as a social worker, psychologist, occupational therapist, or counsellor (AHPRA/AASW registration applicable).
- Experience in acute or community mental health settings.
- Strong assessment and intervention skills.
- Valid NSW driver's licence.
- NSW Working With Children Check.
