**Care Coordinator - Hospital Admission Risk Program**

We have multiple roles (variable EFT) available within the Hospital Admission Risk Program (HARP). These positions are ongoing and require a commitment to work on weekends and public holidays. These roles will be initially based at the Royal Park Campus.

**Your new role**:

- These roles aim to engage with clients through telephone or telehealth modalities to assesses and address risk factors most likely to lead to hospital re-admission. This contributes to the overall HARP aim of improving clients' health outcomes and reduce the risk of unnecessary representations.
- Triaging recently discharged clients who are at high risk of re-presenting to hospital and engage via telehealth / phone to develop appropriate plans of care to address their health, psycho-social issues.
- Ensure clients receive adequate health education / information in an appropriate and accessible format to support their post discharge care needs.
- Actively support clients to make informed decisions about their treatment options and ongoing care needs, including appropriate medical follow up as required.

**About you**:

- Registered Nurse with experience in complex needs assessment, care planning and coordination for clients with chronic disease and complex psychosocial needs
- Understand factors contributing to hospital admission in clients with complex needs
- High level knowledge of Community Services
- Demonstrated ability to be flexible, energetic, innovative, and work cooperatively in a multidisciplinary team as well as autonomously
- Competent IT user, with well-developed written and verbal communication skills
