**We are an Aged Care In-Home Care Provider based in Northcote looking for a part time care coordinator to join our team for 15 hours per week.**

We are seeking a compassionate and experienced Care Coordinator to join our team at Maree Care. As a Care Coordinator, you will play a vital role in ensuring the delivery of high-quality care services for our members within their own homes.

**Responsibilities**:

- Coordinate and manage care services for a caseload of members, ensuring their needs are met effectively and efficiently.
- Maintain accurate documentation and records of client care, including progress notes and care plans.
- Respond promptly to client inquiries, concerns, and emergencies, providing appropriate support and resolution.
- Stay updated with industry trends, best practices, and regulatory requirements related to aged care.

**Requirements**:

- Previous experience in care coordination, case management, or a similar role within the aged care or healthcare sector.
- Excellent interpersonal and communication skills to effectively interact with clients, families, and healthcare professionals.
- Exceptional organizational and time management skills to prioritize tasks and meet deadlines.
- Proficiency in using computer systems and software for documentation and care coordination purposes.
- Valid driver's license and access to reliable transportation.

**Responsibilities**:

- Provide our members with assistance as they accomplish daily tasks, including cleaning, domesitc tasks, shopping, personal care etc.
- Maintain all care records in accordance with company policy
- Communicate any problems, concerns or issues to residents' family members as needed
- Build rapport with members by establishing personal connections by caring and showing interest in their lives
- Respond to any safety or medical issues

Minimum Requirements
- Must be willing to travel
- Police Check or be willing to have a police check
- CERT III (preferred)

**Job Type**: Part-time

**Salary**: $30.00 - $35.00 per hour

**Education**:
