About This Role

We're seeking a skilled professional to play a pivotal role in delivering high-quality care and ensuring our clients receive the best possible support.

Key Responsibilities:
  1. Patient Progress Monitoring: Conduct thorough follow-up assessments to evaluate patient progress and identify areas for improvement, with a focus on preventing readmissions.
  2. Performance Tracking: Generate comprehensive reports on key performance indicators and client outcomes, providing valuable insights for data-driven decision making.
  3. Data Analysis: Analyze trends and patterns in client data to inform service improvements and optimize care delivery.
  4. Referral Management: Coordinate referrals from healthcare professionals or agencies, ensuring seamless transitions in care and services.
  5. Client Record Keeping: Maintain accurate and up-to-date records of client care plans, communication logs, and other relevant information.
Requirements

A Bachelor's degree in Nursing is preferred, but not required. Previous experience in care coordination or a related field is also valuable, as it demonstrates the ability to work effectively with diverse stakeholders and navigate complex care systems.
