Care Coordination and Navigation Role

Eastern Melbourne requires skilled Registered Nurses to deliver care coordination, navigation, and health coaching services via face-to-face or telehealth.

  • Develop tailored care plans, referring to other providers as needed, navigating multi-disciplinary services to fulfill care requirements, facilitating service linkage such as transport, and promoting self-management principles.
  • Provide direct care clinical assessment and intervention aimed at preventing unplanned hospital admissions.

About You:

  • Relevant qualification as a Registered Nurse with experience in delivering chronic condition management, health care navigation, health coaching, and health literacy principles.
  • Excellent communication (verbal, written, and listening) and interpersonal skills for liaising with clients, carers, families, and healthcare professionals.
  • Demonstrated ability to provide education, support, and coaching to clients and their carers.
  • Effective time management and organisational skills to plan and prioritise work efficiently.
  • Ability to work both autonomously and collaboratively within a team to contribute to program objectives.

Requirements:

  • Capability to work independently with minimal supervision.
  • Provision of evidence of vaccination against COVID-19, including booster, or medical exemption.
