Are you a passionate and experienced leader in NDIS Support Coordination, ready to empower a team and enhance participant outcomes? Join Access Support Coordination (ASC) and play a critical role in supporting individuals with disability to achieve their independence and goals

**About the Role**
Access Support Coordination is seeking a dedicated **Support Coordination Team Leader** to lead a team of Support Coordinators / Recovery Coaches. Reporting to the Support Coordination Manager, this full-time role is crucial for ensuring direct reports actively engage services and supports to enhance participant outcomes, meet KPIs, and comply with NDIS quality and safeguard commission standards. You will provide essential support, mentoring, and skill development to ensure high-quality service delivery.

We are open to anyone in the **Southern Qld Region.**

**Key Responsibilities**:

- **Team Leadership & Development**: Lead, mentor, and provide skill development to Support Coordinators/Recovery Coaches, ensuring they meet KPIs and maximise productivity. Conduct regular supervision, debriefing, and Individual Performance Reviews.
- **Participant Outcomes**: Monitor team service delivery to ensure maximum engagement for participants, especially those with complex needs, and support the team in building participant capacity to achieve NDIS goals.
- **Compliance & Quality**: Ensure direct reports undertake safety and support planning assessments and complete relevant documents in collaboration with participants and stakeholders. Adhere to NDIS relevant guidelines and Practice Standards.
- **Stakeholder Collaboration**: Support your team to collaborate and communicate effectively with a range of service providers to strengthen relationships and meet complex participant needs. Participate in maintaining relationships with NDIS, CMOs, and Mental Health Services.
- **Continuous Improvement**: Contribute to identifying areas for improvement and actively participate in implementing change and continual development within the business unit.
- **Reporting & Site Visits**: Report to the Support Coordination Manager on team productivity, highlights, and concerns. Conduct regular site visits to team members' locations as directed.

**About You**:

- Minimum of 2 years of demonstrated experience working within the NDIS, with a combination of relevant experience, education, and training in a similar leadership role.
- Essential: Certificate IV in mental health, community services, or other related health fields.
- Strong understanding of the NDIS framework, comprehensive and historical knowledge of the NDIS program.
- Highly organised with exceptional attention to detail, time management, organisational, and administrative skills.
- Ability to work both independently and collaboratively in a team environment.
- Proven ability to work confidentially and productively with a high level of tact and diplomacy.
- Must hold and maintain a **Working with Children Suitability Blue Card, NDIS Worker Screening, and a Current unencumbered Class C Driver’s Licence.**
