About the Role

VitalCore Support Services is seeking experienced NDIS Support Coordinators to join our contractor network in Brisbane. We specialize in supporting participants with complex mental health conditions, AOD issues, psychosocial disabilities, and comorbidities requiring coordinated, multi-disciplinary intervention.

This is a 
fully remote, flexible arrangement
 with complete autonomy over your schedule and caseload. While services are delivered remotely where possible, you'll conduct participant visits throughout Brisbane and surrounding areas (within 100km radius of Brisbane CBD) as clinically appropriate.

 NDIS Support Coordination Level 2

Key Responsibilities

As an independent Support Coordinator with VitalCore, you will:

• Conduct comprehensive assessments
   of participant needs, strengths, and environmental factors for individuals with complex mental health and AOD presentations
• Link participants to appropriate, evidence-based services
   that align with their NDIS plans and recovery goals
• Build participant capacity
   to understand, implement, and independently manage their NDIS plans
• Coordinate multiple service providers
   and facilitate communication across clinical teams, allied health, and support networks
• Identify and resolve barriers
   to plan implementation, including systemic challenges and service gaps
• Maintain accurate, contemporaneous case notes
   and documentation meeting NDIS Practice Standards and VitalCore requirements
• Liaise with NDIS planners, LACs, families, and stakeholders
   to ensure coordinated care
• Work autonomously
   to manage your caseload, schedule, and service delivery approach within NDIS frameworks

About You

To thrive as an independent contractor with VitalCore, you will bring:

Essential Requirements

• Minimum 1-2 years demonstrated experience
   as an NDIS Support Coordinator (Level 2 preferred)
• Current NDIS Worker Screening Clearance
   (or ability to obtain immediately)
• Working With Children Check
   (Queensland)
• Current driver's license
   and reliable vehicle for participant visits across Brisbane
• Relevant tertiary qualifications
   in social work, psychology, occupational therapy, nursing, counseling, or related disciplines (or equivalent demonstrated experience in disability/mental health support)
• Professional Indemnity Insurance
   ($10M minimum) and 
  Public Liability Insurance
   ($10M minimum)
• Strong understanding
   of NDIS legislation, Practice Standards, Code of Conduct, and operational guidelines
• Your own equipment:
   Computer/laptop, reliable internet connection, mobile phone

Highly Valued Experience

• Working with complex mental health presentations
  , including severe and persistent mental illness, psychosocial disability, and dual diagnosis
• Experience supporting participants with AOD issues
  , substance use disorders, and co-occurring conditions
• Trauma-informed practice
   and understanding of complex trauma presentations
• Knowledge of Brisbane mental health services
  , hospital discharge pathways, and community support networks
• Capacity to work effectively with multiple comorbidities
   and high-complexity participants
