Job Title: Foster Care Case Manager

We are seeking a highly motivated professional to join our team as a Foster Care Case Manager. This challenging and rewarding role will provide you with the opportunity to work with children, birth parents, foster carers, and support services.

Key Responsibilities:

  • Provide case management services to foster care families and young people in care.
  • Assess and manage risk to ensure the safety and well-being of children and young people.
  • Develop and implement individualized plans to meet the needs of children and young people.
  • Build and maintain positive relationships with children, foster carers, birth parents, and other stakeholders.
  • Communicate effectively with staff, clients, and other agencies to ensure seamless service delivery.

Requirements:

  • Bachelor's qualification(s) in Social Work, Social Welfare, Psychology or related fields.
  • Knowledge, skills, and experience working with children and families.
  • Demonstrated ability to write reports to a professional standard.
  • Ability to work independently and as part of a team.
  • Flexible to work some evenings and weekends.
  • Current unrestricted driver's license.
