Case Manager Family Preservation

We are currently seeking two compassionate and skilled Case Managers for our Family Preservation team. You will have the opportunity to work intensively with families to strengthen parenting capacity, enhance safety, and keep children at home wherever possible.
Using trauma informed frameworks, you will partner with families, community supports, and professional networks to achieve sustainable change and positive outcomes.
Key Responsibilities
- Provide intensive, in-home, and community-based case management to families referred under Family Preservation programs.
- Undertake comprehensive child and family assessments, including risk and safety assessments, within required timeframes.
- Develop and implement Family Action Plans and time-oriented, evidence-based case plans in partnership with families, DCJ, and other key stakeholders.
- Deliver evidence-informed interventions that build parenting skills, strengthen family functioning, and address safety concerns.
- Facilitate access to relevant supports such as housing, health, education, counselling, and cultural connections.
- Maintain accurate, timely, and high-quality case records and reports in line with legislative and contractual requirements.
- Participate in reflective supervision, team meetings, and ongoing professional development to ensure continuous practice improvement.
About You
- Diploma or Degree in Social Work, Psychology, Social Science, Community Services, or a related field such as Nursing.
- Demonstrated experience in family preservation, child protection, health care or intensive family support.
- Sound understanding of Family Action Plans and the development of time-oriented, evidence-based case plans.
- Strong understanding of child development, trauma, attachment, and strengths-based practice models.
- Skilled in risk assessment, safety planning, and collaborative case planning.
- Ability to engage families experiencing complex challenges, including domestic and family violence, substance use, mental health issues, and intergenerational trauma.
- Confident in communicating, able to provide advocacy, and most importantly an ability to build positive and sustainable relationships.
- Current NSW Working with Children Check, National Police Check, and unrestricted driver's licence.
