Job Title: Housing Case Manager

Mission Australia is seeking a skilled and compassionate Housing Case Manager to join our team in providing essential support services to individuals and families experiencing homelessness.

About the Role

This is an exciting opportunity to make a meaningful difference in the lives of Australians in need. As a Housing Case Manager, you will work closely with clients to develop and implement personalized case plans, addressing the complex issues associated with homelessness, such as housing tenure, education, and employment opportunities.

Key Responsibilities
  • Conduct initial assessments of suitability for support and develop holistic case plans that cater to clients' cultural strengths and identified needs.
  • Provide intensive ongoing case management sessions, reviewing progress against individual support plans and linking clients to relevant support services.
  • Build professional relationships with clients, maintaining confidentiality and adhering to best practice care standards.
  • Collaborate with internal and external service providers to ensure seamless support and advocacy for clients.
  • Manage a range of case management duties, including referrals, support letters, and appointment setting.
Requirements

To succeed in this role, you will require:

  • A Certificate IV or Diploma in a relevant human/community services field, with at least three years' experience working in the sector.
  • Experience and/or knowledge of issues surrounding homelessness, including alcohol and other drugs, family and domestic violence, unemployment, and mental health.
  • Demonstrated case management experience, including assessment, case work, accurate recording of case notes, advocacy, and referral.
  • Comprehensive knowledge of housing, accommodation, and community service providers, as well as relevant government agencies.
  • Excellent communication and interpersonal skills, including verbal and written communication, report writing, and statistical collection.
  • Ability to work with diverse client cohorts, including individuals, youth, couples, families, women, and children, with a focus on clients experiencing homelessness and related issues.
