Job Title: Housing Case Manager

As a key member of our Housing Support Services, you will work closely with individuals and families who are homeless or at risk of becoming homeless, providing them with the support and guidance they need to secure stable accommodation and achieve their goals.

Key Responsibilities:
  • Conduct initial assessments of suitability for support and develop holistic case plans that address the unique needs and goals of each client.
  • Provide intensive ongoing case management sessions, reviewing progress against individual support plans and linking clients to relevant support services.
  • Build strong relationships with clients, families, and community service providers to ensure effective support and advocacy.
  • Maintain accurate and timely records of case notes, assessments, and progress.
  • Collaborate with other service providers to ensure a comprehensive and coordinated approach to supporting clients.
Requirements:
  • Certificate IV or Diploma in a relevant human/community services field, with at least three years' experience working in the sector.
  • Strong knowledge of issues surrounding homelessness, including alcohol and other drugs, family and domestic violence, unemployment, and mental health.
  • Excellent case management skills, including assessment, case work, and advocacy.
  • Ability to work with diverse client cohorts, including individuals, youth, couples, families, women, and children.
  • Strong communication and interpersonal skills, with the ability to build relationships with clients, families, and community service providers.
