About the Role

We are seeking a skilled and compassionate Housing Case Manager to join our team in [location]. As a Housing Case Manager, you will work with individuals and families who are homeless or at risk of becoming homeless, providing them with the support and guidance they need to secure long-term stable accommodation.

Key Responsibilities
  • Conduct initial assessments of suitability for support and develop holistic case plans that address the client's needs and goals.
  • Provide intensive ongoing case management sessions with clients and review progress against individual support plans.
  • Link clients to relevant support services, including AOD, FDV, health, education, employment, recreation, and social networks.
  • Undertake a range of case management duties, including referrals and support letters, interaction with other service providers, appointment setting, and advocacy.
Requirements
  • Certificate IV or Diploma in relevant human/community services with at least three years' experience working in the sector.
  • Experience and/or knowledge of issues surrounding homelessness, including alcohol and other drugs, family and domestic violence, unemployment, and mental health.
  • Demonstrated case management experience, including assessment, case work, accurate and timely recording of case notes, advocacy, and referral.
  • Comprehensive knowledge and ability to network and build relationships with housing, accommodation, and community service providers and relevant government agencies.
  • Ability to work with a range of client cohorts, including individuals, youth, couples, families, women, and children, and have knowledge of family systems.
