Community Focused Care Coordinator

We are seeking a highly skilled and compassionate Care Coordination Officer to join our team. In this role, you will be responsible for providing vital administrative and coordination support to Care Partners responsible for care planning and support of older adults and people with disabilities in receipt of services under the Commonwealth Home Support Program (CHSP).
The successful candidate will be responsible for ensuring all internal and external customer enquiries are responded to in a timely and customer-focused manner. You will also prepare and send client correspondence, including client information packs, client care plans, agreements, and service information.
You will assist Care Partners with the coordination and commencement of clients' support services, including liaison with internal home and community care teams. Additionally, you will coordinate changes required to clients' regularly scheduled services, escalate client incidents, feedback, and complaints to allocated Care Partners, and maintain accurate and secure assessment, referral, and care and service planning records.
