Lead Care Professional

We provide in-home care and support services tailored to the needs of ageing adults, enhancing their lives and addressing national challenges associated with Australia's ageing population.

Our goal is to deliver exceptional client experiences through effective coordination and management of care activities. This role entails leading and coordinating the overall client experience, collaborating closely with the office team to ensure seamless service delivery.

This position requires a skilled professional to lead service enquiries, care consultations, client onboarding, reviews, and care management. It involves making decisions that prioritize the best interests of clients while supporting business objectives.

Key Responsibilities:

  • Plan, manage, coordinate, and review client goals effectively.
  • Communicate actively with team members and clients to develop comprehensive care plans.
  • Formalize agreements with clients and their families regarding care services and costs.
  • Accurately document all interactions held with clients, families, or other stakeholders about the client and share relevant information with appropriate team members, including feedback on CAREGiver Performance.
  • Meet with clients and their families face-to-face to discuss and review current care plans and goals.

About You:

  • A qualification in a health-related discipline, such as social work, allied health, or a related field, is desirable.
  • Previous relevant experience working with seniors or in aged care is essential.
  • Must have 3-4 years' experience working in a similar role.
  • Highly efficient and organized with proven time management skills.
  • Able to work under pressure and multitask, shifting priorities as needed.
  • Strong written and verbal communication skills.
  • Must hold a valid Australian Driver's License.
