You will be part of a dynamic team working with young carers and their families to help them thrive through connection, opportunity, and skill-building activities. As a Case Manager, you will provide individualised support to help young people overcome challenges and build resilience.

Main Responsibilities:
  • Providing case management services to young carers and their families to promote well-being and resilience
  • Facilitating group activities and school holiday programs to foster fun, skills, and connections
  • Linking young people with services, supports, and opportunities that enhance their lives
  • Advocating for young carers and helping them navigate life's challenges with confidence and dignity
  • Promoting the program at schools and community events to raise awareness and build partnerships

To succeed in this role, you will bring a degree in Social Work, Psychology, or Community Services (or Diploma + extensive experience), along with:

  • Experience in case management, youth work, or community services
  • A trauma-informed, culturally inclusive approach that prioritises respect, safety, and empowerment
  • Strong communication, group facilitation, and advocacy skills that foster positive relationships and outcomes
  • The ability to work collaboratively with diverse stakeholders, including young people, families, and community organisations
