Job Title: Service Coordinator

As a Service Coordinator within our Home Health program, you will play a vital role in managing a portfolio of customers and a team of Care Workers in the Tweed Heads area. Your primary responsibility will be to ensure seamless service delivery, foster a high-performing team culture, and maintain a strong focus on compliance.

Key Responsibilities:
  • Conduct recruitment activities and onboard new Care Workers.
  • Provide dedicated client management, including initial assessments, annual reviews, and comprehensive care planning.
  • Manage and resolve client escalations and complaints to ensure high levels of satisfaction.
  • Drive customer acquisition and growth while building a positive brand presence in the community.
  • Build and develop valuable relationships with external service providers and internal Clinical and Allied Health teams.
Requirements:
  • Proven people leadership experience with a track record of cultivating high-performing teams.
  • Exceptional multitasking and ability to take ownership of customer-focused solutions.
  • Prior experience in managing financial budgets and working to KPIs and targets.
  • Strong stakeholder management and ability to drive business outcomes.
  • Fully COVID-19 vaccinated with 2 doses (Booster is optional).
