About the Role

Australian Unity is seeking a skilled Clinical Care Coordinator to join our team in delivering the Diabetes Using Our Strengths Service (DUOSS) pilot research program. The DUOSS is an innovative virtual and face-to-face service that incorporates remote patient monitoring, optimizing management of diabetes medicines, education, and group yarn ups to help Aboriginal and Torres Strait Islander peoples manage their type 2 diabetes.

As a Clinical Care Coordinator on this program, you will lead the clinical delivery of the Diabetes Using Our Strengths Service (DUOSS). The role will work in a digitally enabled model of care and be responsible for the delivery of quality, clinical care for patients in managing their type two diabetes in a holistic and culturally appropriate way.

The successful candidate will deliver technology-enabled, culturally appropriate care, including face-to-face and remote patient monitoring, evidence-based chronic disease management, care coordination, clinical escalation of any concerning results, signs, or symptoms, and health coaching.

About You

You will have a demonstrated ability to communicate sensitively and effectively with Aboriginal and/or Torres Strait Islander people and to facilitate culturally appropriate group yarning sessions. You will also have a Registered Nurse qualification with unrestricted AHPRA registration and community nursing experience, as well as clinical skills in chronic disease management and delivering care within a multi-disciplinary framework.
