Role Overview

The National Care Coordinator plays a pivotal role within the Mission, Vision, and Values of Cocoon Group Services. This position is tasked with orchestrating care and offering clinical direction to a multidisciplinary team, including Service Managers, Team Leaders, Registered and Enrolled Nurses, and Disability Support Workers. The goal is to ensure the delivery of exemplary clinical care to residents and participants. Additionally, the National Care Coordinator is responsible for training and supervising designated staff to address the intricate care requirements of residents.

Key Responsibilities:

  • Contribute actively to the Care/Support Team and Compliance Team.
  • Provide clinical insights to enhance the effectiveness of the Care/Support Team and Compliance Team.
  • Identify service gaps and spearhead continuous improvement initiatives to uphold clinical best practices.
  • Foster effective communication and strong relationships with Residents, Participants, and Support/Care Team Members.
  • Develop a proficient Care/Support Team through coaching, mentoring, training, and conducting competency evaluations.
  • Encourage a service delivery culture that respects the choices and independence of Participants and Residents.
  • Support the implementation of Cocoon Group Services' clinical governance framework.
  • Assist Residents and Participants with complex needs through adaptable case management aligned with their goals.
  • Oversee High Intensity Support Plans and assessments.
  • Facilitate Resident or Participant re-assessments and provide ongoing support to all stakeholders.
  • Manage clinical incidents, risks, and complaints raised by staff and external parties.
  • Conduct home visits to ensure clinical compliance and quality standards.
  • Assist Residents or Participants in preparing for NDIS Review Meetings, including nursing assessments.
  • Maintain and enhance professional knowledge and skills in line with regulatory requirements.
  • Review and develop clinical policies, procedures, and workflows, participating in internal and external audits.

Qualifications and Requirements:

  • Current unrestricted Registered Nurse Registration as per the Australian Nurses and Midwifery Board.
  • NDIS Worker Screening Check.
  • Completion of NDIS Mandatory Worker Orientation Certificate.
  • Working with Children Check (WWCC) clearance obtained within the last five years.
  • COVID-19 Infection Control Certificate.
  • Current COVID-19 vaccination documentation.
  • First Aid, CPR, and Anaphylaxis Training.
  • 100 points of identification.
  • Right to work in Australia.
  • Valid Driver License and associated Insurance.
