Role Overview

The National Care Coordinator plays a pivotal role within Cocoon Group Services, ensuring the delivery of exceptional clinical care. This position involves providing clinical oversight and guidance to a multidisciplinary team, including Service Managers, Team Leaders, Registered and Enrolled Nurses, and Disability Support Workers. The aim is to uphold the highest standards of care for residents and participants.

Key Responsibilities:

  • Actively contribute to the Care/Support Team and Compliance Team.
  • Deliver clinical expertise to enhance team performance.
  • Identify areas for service enhancement and lead initiatives for continuous improvement.
  • Foster effective communication and build strong relationships with Residents, Participants, and team members.
  • Facilitate the development of a skilled Care/Support Team through coaching, mentoring, and training.
  • Encourage a service delivery culture that prioritizes Participant and Resident choice and independence.
  • Support the implementation of the clinical governance framework.
  • Assist Residents and Participants with complex needs through tailored case management.
  • Oversee High Intensity Support Plans and conduct assessments.
  • Coordinate clinical incidents, risks, and complaints in collaboration with stakeholders.
  • Conduct home visits to ensure clinical compliance and quality of care.
  • Assist Residents in preparing for NDIS Review Meetings, including nursing assessments.
  • Maintain and enhance professional knowledge in line with regulatory requirements.
  • Review and develop clinical policies and participate in audits.

Qualifications and Requirements:

  • Current unrestricted Registered Nurse Registration with AHPRA.
  • NDIS Worker Screening Check.
  • Completion of NDIS Mandatory Worker Orientation.
  • Working with Children Check (WWCC) clearance.
  • COVID-19 Infection Control Certification.
  • Current COVID-19 vaccination documentation.
  • First Aid, CPR, and Anaphylaxis Training.
  • Proof of identity (100 points).
  • Eligibility to work in Australia.
  • Valid Driver License and insurance.
