National Care Coordinator

Cocoon Group Services is seeking a highly skilled and experienced National Care Coordinator to join our team. As a key member of our clinical governance team, you will be responsible for coordinating care and providing clinical guidance to our teams to ensure the provision of high-quality, best-practice clinical care to our residents and participants.
Key Responsibilities- Clinical Governance: Provide clinical advice to our care and support teams to ensure compliance with legislative and statutory requirements, and NDIS high-intensity skill descriptors.
- Care Coordination: Coordinate care and provide clinical guidance to our teams to ensure the provision of high-quality, best-practice clinical care to our residents and participants.
- Training and Supervision: Provide training and supervision to our support workers to ensure they have the necessary skills and knowledge to provide high-quality care.
- Quality Assurance: Assist in identifying service gaps and improvements and lead ongoing continuous improvement to achieve clinical best practice.
- Communication and Relationship Building: Communicate effectively and build strong relationships with our residents and participants, as well as other support and care team members.
- Culture of Service Delivery: Promote a culture of service delivery that supports the participant's choice and independence.
- Implementation of Clinical Governance Framework: Support the implementation of our clinical governance framework to ensure compliance with legislative and statutory requirements.
- Complex Needs and Behavioural Concerns: Support our residents and participants with complex needs and behavioural concerns through responsive and flexible case management aligned with resident and participant goals and continual support towards recovery and positive outcomes.
- High-Intensity Support Plans and Assessments: Oversight of high-intensity support plans and assessments to ensure compliance with NDIS requirements.
- Clinical Incidents, Risks, and Complaints: Coordinate clinical incidents, risks, and complaints raised by staff, external stakeholders, and the NDIS Quality and Safeguards Commission.
- Home Visits and Clinical Compliance: Visit our residents and participants in their homes and check clinical compliance and quality.
- NDIS Review Meetings: Support our residents and participants to prepare for their next NDIS review meeting, including preparation of nursing assessments and reports.
- Professional Development: Maintain professional knowledge and continuing to develop knowledge and expertise in relation to professional skills and regulatory requirements.
- Clinical Policies, Procedures, and Processes: Review and development of clinical policies, procedures, and processes, and participation in NDIS internal and external audits.
- Current Unrestricted Registered Nurse Registration: Current unrestricted registered nurse registration as approved by the Australian Nurses and Midwifery Board.
- NDIS Worker Screening Check: Current NDIS worker screening check.
- NDIS Mandatory Worker Orientation Certificate: NDIS mandatory worker orientation certificate of completion.
- Working with Children Check: Working with children check clearance within the last five years.
- COVID-19 Infection Control Certificate: Current COVID-19 infection control certificate.
- COVID-19 Vaccination Certificates: Current COVID-19 vaccination certificates.
- First Aid, CPR, and Anaphylaxis Training: First aid, CPR, and anaphylaxis training.
- 100 Points of Identification: 100 points of identification.
- Right to Work in Australia: Right to work within Australia.
- Current Driver License and Insurance: Current driver license and associated insurance.
