Cocoon Group Services is seeking a highly skilled and experienced National Care Coordinator to join our team. As a key member of our clinical governance team, you will be responsible for coordinating care and providing clinical guidance to our teams to ensure the provision of high-quality, best-practice clinical care to our residents and participants.
Key Responsibilities- Clinical Governance: Provide clinical advice and guidance to our care teams to ensure compliance with legislative and statutory requirements, and NDIS high-intensity skill descriptors.
- Care Coordination: Coordinate care and provide support to our residents and participants with complex needs and behavioral concerns.
- Quality Assurance: Conduct regular quality assurance audits and provide recommendations for improvement.
- Training and Development: Provide training and supervision to our care teams to ensure they have the necessary skills and knowledge to provide high-quality care.
- Compliance: Ensure that all care teams are aware of and comply with relevant laws, regulations, and policies.
- Clinical Incidents: Investigate and manage clinical incidents, risks, and complaints raised by staff, external stakeholders, and the NDIS Quality and Safeguards Commission.
- Visits and Assessments: Conduct regular visits to our residents' homes to check clinical compliance and quality, and conduct assessments and re-assessments as required.
- NDIS Reviews: Support our residents and participants to prepare for their next NDIS review meeting, including preparation of nursing assessments and reports.
- Professional Development: Maintain professional knowledge and continuing to develop knowledge and expertise in relation to professional skills and regulatory requirements.
- Current unrestricted Registered Nurse Registration as approved by the Australian Nurses and Midwifery Board.
- Current NDIS Worker Screening Check.
- NDIS Mandatory Worker Orientation Certificate of Completion.
- Working with Children Check (WWCC) clearance within the last five (5) years.
- COVID-19 Infection Control Certificate.
- Current COVID-19 vaccination certificates.
- First Aid, CPR, and Anaphylaxis Training.
- 100 points of identification.
- Right to work within Australia.
- Current Driver License and associated Insurance.
