Role Overview
The National Care Coordinator plays a pivotal role within the Mission, Vision, and Values of Cocoon Group Services. This position is tasked with the coordination of care and the provision of clinical guidance to a multidisciplinary team, including Service Managers, Team Leaders, Registered and Enrolled Nurses, and Disability Support Workers. The goal is to ensure the delivery of high-quality, best-practice clinical care to residents and participants. Additionally, the National Care Coordinator is responsible for training and supervising designated staff to meet the complex care needs of residents.
Key Responsibilities:
- Actively participate as a member of the Care/Support Team and Compliance Team.
- Provide expert clinical advice to enhance the effectiveness of the Care/Support Team and Compliance Team.
- Identify service gaps and drive continuous improvement initiatives to achieve clinical excellence.
- Foster effective communication and build strong relationships with Residents, Participants, and Care Team Members.
- Develop a competent Care/Support Team through coaching, mentoring, training, and conducting competency assessments.
- Encourage a service delivery culture that prioritizes the choice and independence of Participants and Residents.
- Support the implementation of Cocoon Group Services' clinical governance framework.
- Assist Residents and Participants with complex needs through responsive case management aligned with their goals.
- Oversee High Intensity Support Plans and conduct assessments.
- Facilitate re-assessments and reviews for Residents or Participants, providing ongoing support to all stakeholders.
- Coordinate clinical incidents, risks, and complaints raised by staff and external stakeholders.
- Conduct home visits to ensure clinical compliance and quality standards.
- Assist Residents or Participants in preparing for NDIS Review Meetings, including the preparation of nursing assessments/reports.
- Maintain and enhance professional knowledge in relation to clinical skills and regulatory requirements.
- Review and develop clinical policies, procedures, and workflows, participating in internal and external audits.

Qualifications and Requirements:
- Current unrestricted Registered Nurse Registration with the Australian Nurses and Midwifery Board (AHPRA).
- Current NDIS Worker Screening Check.
- Completion of NDIS Mandatory Worker Orientation Certificate.
- Working with Children Check (WWCC) clearance within the last five years.
- COVID-19 Infection Control Certificate.
- Current COVID-19 vaccination certificates.
- First Aid, CPR, and Anaphylaxis Training.
- 100 points of identification.
- Right to work in Australia.
- Current Driver License and associated Insurance.
