Homeless Support Case Worker Role
We are seeking a skilled and compassionate Case Manager to join our team. This challenging role involves working closely with clients at risk of homelessness, providing quality case management services to help them find and sustain safe, affordable housing.
About the Job:
This is a dynamic position that requires strong communication and organizational skills, as well as the ability to work effectively in a team environment. As a Case Manager, you will develop and implement personalized case plans to address client needs, provide guidance on accessing appropriate accommodation, and ensure accurate and timely records and reports.
Key Responsibilities:
- Develop case plans to address client needs and goals
- Guide clients to access suitable accommodation and support services
- Maintain accurate and up-to-date records and reports
- Provide referrals to employment, training, and other relevant services
Qualifications and Experience:
- A relevant tertiary qualification (e.g., Diploma of Community Services Work or Bachelor of Community Welfare)
- Minimum 4 years of experience in case management, preferably in the community sector
- Demonstrated ability to work with individuals experiencing mental health issues and trauma
