Job Overview

The Coordinated Access Manager is a key role in the implementation and advancement of Coordinated Access within the Cape Breton Regional Municipality (CBRM). This position is responsible for setting up, managing, promoting, adopting, and developing the Coordinated Access system to serve individuals who are homeless or at risk of homelessness.

About the Position

Funded by the CBRM Reaching Home Community Entity, Coordinated Access aims to bring together organizations that serve individuals who are homeless or at risk of homelessness. The ultimate goal is to achieve community objectives such as reducing chronic homelessness, homelessness overall, and returns to homelessness.

The Coordinated Access Manager serves as the secretariat to the Governance committee and works closely with the committee and other stakeholders to examine and accomplish the objectives laid out in the Reaching Home Coordinated Access Guide.

Duties and Responsibilities
  • Governance Committee Management:
  • Coordinate regular meetings of the local governance committee
  • Ensure committee representation includes all relevant stakeholders
  • Recommend new members as needed and onboard them effectively
  • Support timely decision-making to advance CA program goals
  • Maintain up-to-date Terms of Reference (Roles and Responsibilities) for the committee
  • Educate committee members on clear coordinated access outcomes, local priority populations, and 12-month governance committee goals
  • HIFIS Implementation and Compliance:
  • Guarantee effective implementation and compliance among primary users
  • Broaden understanding and adoption among secondary users through training and education
  • Create visual materials to enhance understanding and promote broader adoption
  • Build trust in HIFIS and its management among users
  • Provide assistance with troubleshooting and resolving user issues
  • System Map Creation and Maintenance:
  • Create a comprehensive system map of services for homeless/at-risk populations
  • Develop the map as a precursor to future Coordinated Access Shared Resources Inventory
  • Making the map accessible online, searchable, and designed for community users
  • Data Management, Analysis, and Sharing:
  • Establish and maintain policies and protocols for data privacy and safety
  • Respond promptly to data breaches and implement measures to prevent future incidents
  • Develop user guides to enhance data security and privacy practices
  • Analyze and share relevant data with the governance committee, stakeholders, community, municipality, and province to inform strategies for eliminating homelessness
  • Access Points, AP Protocols, and Shared Assessment Tools:
  • Collaborate with the local governance committee to establish and promote CA access points and protocols
  • Work towards a set of shared assessment and triage tools for efficient service delivery
  • Develop a coordinated system that meets local needs and maximizes the potential of Coordinated Access in the CBRM
