Community Support Professional

As a Community Support Professional, you will work within a strength-based context with holistic principles to build individual capacity, resilience, and well-being.

Key Responsibilities:

  • Assess community members' specific needs, strengths, and identification of interrelated issues and needs.
  • Develop high-quality case plans with community members to identify achievable goals using a strength-based approach.
  • Support community members to build and increase capacity and develop independent living skills towards self-determination.
  • Develop and maintain referral pathways for other support and services.
  • Provide advocacy to support community members to access services and be connected in their community and be linked to other communities as appropriate.

Requirements:

Relevant Degree qualification in Social Work, Psychology, or other Community/Human Services discipline.

A current and valid Working with Children's Check.
