Australian Unity is seeking a highly skilled and experienced Service Coordinator to join our team. As a key member of our Home Health program, you will be responsible for managing a portfolio of approximately 150 customers and a team of Care Workers.
About the Role:
- Customer Acquisition and Growth: Drive customer acquisition and growth while building a strong and positive brand in the community.
- Client Management: Provide dedicated client management by conducting initial assessments, annual reviews, and developing comprehensive care plans.
- Escalation Management: Manage and resolve client escalations and complaints to ensure high levels of satisfaction.
- Relationship Building: Build and develop valuable relationships with various external service providers and internal Clinical and Allied Health teams.
- Recruitment and Development: Drive a forward-thinking recruitment strategy and support your team throughout the employment lifecycle.
- Culture and Compliance: Promote a culture of safety and lead by example, while maintaining a strong focus on compliance within your portfolio.
About You:
- Experience: Prior Aged Care Case Management experience is highly desirable, but not essential.
- Leadership: Proven People Leader with a track record of cultivating a high-performing and engaged team.
- Coaching and Development: Strong coaching and development capabilities, using both formal and informal methods to nurture your team's growth.
- Financial Management: Prior experience in managing financial budgets.
- Performance Management: Proven success rate in working to KPIs and targets.
- Stakeholder Management: Effective stakeholder management with the ability to drive business outcomes.
- Organisation and Technology: Strong organisation skills with the ability to self-manage your priorities, and high proficiency in Microsoft Suite and CRMs.
