Service Coordinator

As a Service Coordinator within our Home Health program, your role will involve managing a portfolio of customers and a team of Care Workers in a dynamic and supportive environment.
We are seeking an experienced professional with a strong understanding of Aged Care funding, the Aged Care Quality Framework, and prior Case Management experience.
Key Responsibilities:
- Lead customer acquisition and growth while building a strong and positive brand in the community.
- Provide dedicated client management by conducting initial assessments, annual reviews, and developing comprehensive care plans.
- Manage and resolve client escalations and complaints to ensure high levels of satisfaction.
- Build and develop valuable relationships with external service providers and internal Clinical and Allied Health teams.
- Drive a forward-thinking recruitment strategy and support your team throughout the employment lifecycle.
- Promote a culture of safety and lead by example.
- Drive continuous improvement initiatives and maintain a strong focus on compliance within your portfolio.
Requirements:
- Prior Aged Care experience is essential, including a knowledge of funding levels and Aged Care Quality Standards.
- Proven People Leadership skills with a track record of cultivating a high-performing and engaged team.
- Strong coaching and development capabilities, using both formal and informal methods to nurture your team's growth.
- Exceptional multitasking and the ability to take ownership of customer-focused solutions.
- Prior experience in managing financial budgets.
- Success rate in working to KPIs and targets.
- Effective stakeholder management with the ability to drive business outcomes.
- Strong organisational skills with the ability to self-manage your priorities.
