Job Title: Service Coordinator

As a Service Coordinator within our Home Health program, you will play a vital role in managing a portfolio of customers and a team of Care Workers in Port Macquarie. Your primary responsibility will be to lead customer acquisition and growth while building a strong and positive brand in the community.

Key Responsibilities:
  • Conduct initial assessments, annual reviews, and develop comprehensive care plans to ensure high levels of satisfaction.
  • Manage and resolve client escalations and complaints in a timely and professional manner.
  • Build and develop valuable relationships with external service providers and internal Clinical and Allied Health teams.
  • Drive a forward-thinking recruitment strategy and support your team throughout the employment lifecycle.
  • Promote a culture of safety and lead by example.
  • Drive continuous improvement initiatives and maintain a strong focus on compliance within your portfolio.
Requirements:
  • Prior Aged Care experience, including knowledge of funding levels and Aged Care Quality Standards.
  • Proven People Leader with a track record of cultivating a high-performing and engaged team.
  • Strong coaching and development capabilities, using both formal and informal methods to nurture your team's growth.
  • Exceptional multitasking and the ability to take ownership of customer-focused solutions.
  • Prior experience in managing financial budgets.
  • Success rate in working to KPIs and targets.
  • Effective stakeholder management with the ability to drive business outcomes.
  • Strong organisation skills with the ability to self-manage your priorities.
