This role is responsible for effective delivery and supervision of the high-quality care to youth who are at risk of homelessness. This includes case management assistance using a strengths-based approach, partnering with clients to effectively meet their individual needs and work towards independence where appropriate, coordinating key processes and preparing regular management reports.

Programs overseen by a Case Manager HCS role includes:
**Outreach**

Outreach support includes general outreach, early intervention and assertive outreach. Using a harm minimisation approach, Outreach builds relationships, provide social assistance, supported referrals and other appropriate measures to address complex and multifaceted support needs of people. A key focus is on building and fostering a collaborative approach amongst agencies delivering services or otherwise working with clients.

**Youth Services**

Youth Services provides crisis supported accommodation with case management, for people aged 16-24 years old, who are homeless or at risk of homelessness. Our services provide support and assistance with education, employment, referral into counselling services, health services, living skills and parenting programs.

As the Case Manager, your role includes but is not limited to:

- Work in collaboration with other internal and external services to address rough sleeping
- Conduct regular street outreach with other service providers whilst managing satellite outreach provisions within key partner agencies in Parramatta and Cumberland LGA.
- Promote, engage and grow community development activities including with schools and at community events to raise awareness of homelessness and services available.
- Provide developmentally appropriate and responsible case management service to clients
- Work with clients to develop, implement and review a Case Management Plan

**Essential Criteria**:
**Qualification**:

- Certificate IV in Community Services, Mental Health, Youth Work or other relevant qualification, or equivalent and relevant to the field of work

**Experience**:
Typically this role will require 3 or more years’ experience in your field of expertise. It is expected that you will possess good skills at navigating a complex organisation, forging relationships, and managing through influence rather than direct authority as required.
- Demonstrated skills in time management, setting priorities and being able to plan and organise workflows
- Strong administration skills
- Ability to work autonomously as well as part of the larger team
- Knowledge best-practice guidelines for Specialist Homelessness Services funded services
- Demonstrated engagement with complex clients
- Demonstrated experience liaising with other professionals to develop partnerships and referral pathways
- Driver’s Licence
