We are seeking a full or part time Specialist Support Coordinator with hybrid working arrangements. There will be some mandatory attendance in our Warragul Head Office.

As a Specialist Support Coordinator, you will play a pivotal role in assisting participants with complex needs. Your expertise will help reduce complexities in their support environment, enabling them to overcome significant barriers to plan implementation. This is a challenging yet rewarding opportunity to make a real impact on the lives of those we support.

Key Responsibilities:
Identify and address complex barriers that affect participants' access to and maintenance of supports.

Collaborate with participants, their formal and informal networks to design comprehensive service plans tailored to their needs.

Ensure cohesive operation of all services within a participant's support plan, fostering effective communication and collaboration among all stakeholders.

Provide expert guidance and support to participants and their support networks, ensuring consistent service delivery and access to relevant support during crises or unplanned events.

**Requirements**:
A Bachelor's degree (or higher) in a relevant discipline.

A solid track record in a similar role, with robust case management and support coordination experience.

In-depth understanding of the NDIS framework and a history of working with its participants.

Valid NDIS Worker Screening, Working with Children Check, and Driver's License.

**Job Types**: Full-time, Part-time

**Salary**: $36.00 - $42.00 per hour

Expected hours: No more than 38 per week
