
Support Coordination Specialist

At GexaCare, we are seeking a skilled and experienced Support Coordination Specialist to join our team.
This newly created role offers the opportunity to establish and grow Support Coordination services within our organisation. You will hold responsibility for a small caseload of up to 15 participants, providing specialist support coordination for people with complex needs.
As the first Support Coordinator at GexaCare, you will be expected to actively build and manage your caseload, develop strong referral pathways, and set high standards of service delivery.
The ideal candidate will have:
- A tertiary qualification in psychology, occupational therapy, social work, allied health, developmental or health science.
- Registration with a relevant professional body.
- Minimum two years' experience providing Level 3 Support Coordination or similar services for people with complex needs.
- Strong understanding of the NDIS, service systems and compliance requirements.
You will also have:
- Proven ability to establish referral pathways and build caseloads.
- High-level interpersonal, negotiation and advocacy skills.
- Strong organisational skills with the ability to manage competing priorities.
