Practice Improvement Specialist

About the Role
We are seeking a Practice Improvement Specialist to join our Senior Services team in the Greater Western Sydney area. This role is pivotal in embedding person-centred care principles and driving service model improvements across our programs. You will work closely with operational support and service delivery teams to enhance customer outcomes and promote sustainable change.
Key Responsibilities
- Coach and guide teams in adopting Uniting's Senior Services models of care.
- Lead local practice improvement initiatives and embed sustainable changes.
- Support efficient workflows aligned with service model principles.
- Conduct program and process reviews to support person-centred practice
- Drive capability development and capacity building across services.
- Use feedback to inform service design and improve customer experience.
- Collaborate with shared services and stakeholders to ensure high-quality, safe services.
About You
You are a strategic thinker and change leader with a passion for improving service delivery. You bring:
- 5+ years of experience in coaching, change management, or service improvement.
- A bachelor's degree in a relevant field or equivalent experience.
- Strong communication, analytical, and organisational skills.
- Proven ability to influence and lead transformational change.
- A customer-focused mindset and collaborative approach.
Desirable:
• LEAN, business process improvement, or service redesign experience.
• Postgraduate qualifications in Coaching or Change Management.
• Allied Health or Clinical qualifications (e.g. Nursing).
