Support at Home Care Partner Administrator
is the backbone of our care coordination — ensuring participants funded under the Department of Health and Aged Care receive dignified, timely, and person-centred services that help them thrive at home.

This is a
12-month contract role to cover maternity leave
, offering the chance to make a genuine impact while gaining valuable experience in the aged care sector.

What You'll Do

  • Act as the first point of contact for participants, families, and providers with warmth and professionalism.
  • Coordinate services and equipment orders through our digital platform ensuring seamless communication across teams and external providers.
  • Monitor help plans, budgets, and participant needs to keep services on track and compliant with the Aged Care Act 2024.
  • Liaise with health professionals, third-party providers, and internal care teams to advocate for participants' needs.
  • Manage feedback and complaints with diplomacy and follow through to resolution.
  • Support onboarding and exit processes via My Aged Care and PRODA, ensuring every participant journey is handled with care.
